Care Coordinator

We are looking for experienced care coordinators to manage patient care plans and act as the primary liaison between families and our clinical team at our Kottayam head office.

• Assess patient needs and develop personalised care plans in consultation with nurses and therapists
• Schedule and monitor home-visit assignments for care attendants, nurses, and physiotherapists
• Communicate care updates, incident reports, and plan revisions to families
• Ensure documentation, compliance, and quality standards are met for every case

A background in nursing, social work, or healthcare management with 2 or more years of coordination experience is required. Strong interpersonal and organisational skills essential. Office-based role with occasional field visits.
