Understand the situation
Listen to the person and family, clarify priorities and identify what is already in place.
Get help with daily care or nursing, or with organising the care already in place. A professional Primacy coordinator helps agree a care plan, connect services and follow up actions with consent. Hands-on care and coordination are separate services; scope and availability are agreed before starting.
For people and families managing care—nearby, elsewhere in India or overseas—not only older people or NRI families.
One family. One care plan. One professional coordinating the care.

Local understanding. One agreed plan. Follow-through you can reach.
Each package includes direct and indirect coordination time. Exact scope, service hours, availability and terms are confirmed in writing before service starts.
A professional in your corner
Your coordinator works with the person receiving care and their family to organise agreed support, recognise potential concerns, and follow up actions within the agreed scope. Caregivers, appointments, healthcare providers and family communication are brought together around one care plan.
Listen to the person and family, clarify priorities and identify what is already in place.
Record agreed goals, contacts, next steps and review points; update the plan as needs change.
Arrange check-ins at the agreed frequency, recognise potential concerns and follow up or raise them with the agreed contacts within the service scope.
Coordinate caregivers, appointments, nursing, rehabilitation and other providers with consent.
Help organise practical hospital-to-home arrangements and follow up agreed discharge actions.
Share updates with authorised family members, maintain coordination records and track actions.
The person receiving support remains at the centre of decisions. Case Management does not include diagnosis, replace the treating team, guarantee outcomes or respond to emergencies. Clinical decisions remain with qualified treating professionals; support, review frequency and escalation contacts are agreed before starting.
Support for the whole care picture
Older people living independently who want a reliable point of contact.
Families living elsewhere in Kerala or balancing work and care responsibilities.
Children living in another Indian state or overseas.
People returning home from hospital and families navigating changing needs.
Families coordinating several caregivers, health services or community supports.
This fictional example shows how agreed support could be organised. It contains no real patient information. Your own plan would be agreed with the person receiving support and, with permission, their family. Actions, contact arrangements and review frequency depend on the agreed scope, package hours and availability. This example is not a promise that every action is included in every package.
Responsible: coordinator to contact the provider with permission; authorised family contact to confirm transport arrangements.
Target: [agreed date]. Status: awaiting provider confirmation.
Review point: confirm the appointment details and who will attend; if unavailable, discuss alternatives with the person and authorised contact. Attendance, transport and provider fees are not automatically included.
Responsible: coordinator to confirm agreed arrangements with the caregiver or provider; caregiver or provider remains responsible for delivering their service.
Target: [agreed date]. Status: awaiting provider reply.
Review point: record whether arrangements are confirmed and who will follow up any outstanding issue. Staffing availability is not guaranteed.
Responsible: coordinator to send the permitted action summary to the authorised contact through the agreed channel.
Target: [agreed update date or frequency]. Status: planned.
Review point: check whether actions remain outstanding and whether the person wants any changes to the plan or information-sharing arrangements.
At [agreed review date], discuss progress against the person's goals, completed and outstanding actions, changes in preferences or support needs, permissions, and whether the agreed scope and package hours remain suitable. Record the agreed changes, action owners and next review point. Additional services or coordination time require prior discussion, availability and agreed terms; third-party costs remain separate unless expressly included in a written agreement.
The coordinator organises and follows up agreed support; they do not diagnose, replace treating professionals or guarantee health or service outcomes. Routine concerns are raised with the agreed contacts within the agreed service arrangements. This is not continuous monitoring, an around-the-clock response or an emergency service. For an immediate medical or safety concern, contact local emergency services and the treating team; do not wait for a coordinator reply.
A useful distinction
Your package provides a dedicated amount of professional coordination time each month. Not all care coordination happens face-to-face: your coordinator may be working for the individual even when you are not speaking directly.
Hours belong to the individual and are used for agreed needs under the care plan. They are a monthly ceiling, not an unlimited support entitlement.

The Golden Era Assessment is free for non-urgent planning of future care needs. Urgent care and ongoing care or coordination services are separate; their scope and any charges are discussed before proceeding. The conditional onboarding fee below is for Support Coordination, not this free future-planning assessment.
Why Primacy?
Our Support Coordinators are professional social workers who understand people, families, care systems, dignity and choice.
Coordinators receive training through Primacy Academy, incorporating Australian-informed approaches to care coordination, communication and person-centred practice.
A central coordinator helps keep caregivers, services, family communication and agreed actions connected.
Professional coordination with an understanding of Kerala’s healthcare, family and community environment.
The individual remains in charge of their choices, with support that can connect to in-home care when appropriate.
One Person. One Care Plan. One Point of Contact.
You may live five minutes away, in another city, overseas — or simply be balancing work, children and your own responsibilities. Distance is not the only reason families need help. Sometimes there are simply too many appointments, calls, caregivers and decisions to keep track of. Primacy gives you someone whose role is to help keep those pieces connected.
The practical details, clearly explained.
Hours may be used for direct conversations and agreed meetings, as well as indirect work undertaken specifically for the individual, such as provider liaison, appointment coordination, records, care-plan updates and follow-up. The work is guided by the agreed care plan and package.
No. Monthly coordination hours reset each billing month and do not accumulate or roll over.
No. There is no obligation to use every hour. Coordination is provided according to the person’s needs and agreed care plan, up to the package’s monthly allocation.
Additional time may be arranged subject to availability. Please speak with our team; no additional hourly rate is published on this page, and any terms will be discussed before work is agreed.
Home visits depend on the package, agreed plan, location and requirements. Travel or additional service charges may apply and will be discussed beforehand. Visits are not unlimited.
No. Package fees cover the professional coordination service. Caregiver, nursing, medical, allied-health, transport, equipment and other third-party costs are separate unless expressly stated in a written agreement.
The person’s wishes and consent guide communication. The authorised recipients, information shared, channel and frequency are agreed in the care plan. We do not replace the family or treating professionals.
No. Service hours, availability and response arrangements are confirmed in writing. Priority coordination is not unlimited and this is not an emergency service. For an immediate medical or safety concern, contact local emergency services and the treating team.
A conversation, not a commitment
Share only what is needed to begin. We will listen, check location and availability, and explain the next step. Please do not include detailed medical records or diagnoses in your enquiry.
This is not an emergency line and does not provide immediate response. For an immediate medical or safety concern, contact local emergency services and the person’s treating team.
