Immediate Safety & Stability

When Care Cannot Wait We're Here Now

When needs change suddenly, contact the team to discuss an assessment. Response timing, professional roles, clinical scope, and suitable staff availability must be confirmed.

When care is needed now, you are not alone.

Our Commitment

Safety, Stability & Dignity During Crisis

For a sudden change, hospital discharge, or increasing care need, the team can review an enquiry and explain whether an assessment and suitable staff are available.

Any proposed urgent, high-need, or transition support is subject to assessment, treating-clinician instructions where relevant, a written care plan, and current availability.

Urgent requests reviewed against current availability
Clinical support subject to assessment and availability
Discharge-instruction review subject to consent
Dignified care in familiar surroundings

Support Through Every Transition

Hospital-to-home support depends on discharge instructions, an agreed plan, and suitable availability.

Immediate Care FAQs

Stream 1 is an enquiry pathway for sudden changes in care needs, possible Supported Independent Living, hospital-to-home planning, short-term support, or nursing enquiries. Suitability, clinical scope, staffing, and availability require assessment.

After assessment, a proposal may discuss rostered living support, short-term support, discharge-instruction review, or nursing tasks. Every duty, professional role, schedule, treating-clinician instruction, and suitable staff availability must be confirmed in writing.

Call or WhatsApp the care team as early as possible. Urgent support depends on the person's needs, exact location, and suitable staff availability; no fixed response or placement time is guaranteed.

We can discuss transition support with the family and review clinician-provided discharge instructions. Any medication or wound-care task depends on the treating clinician's instructions and the proposed worker's confirmed qualifications and scope; no outcome or reduction in readmission is guaranteed.

"For any urgent or planned enquiry, confirm duties, timing, escalation contacts, handover, and current availability in writing."
